Service Description

The Wellness Recovery Action Plan (WRAP) is an evidence-based program designed to help individuals manage their mental health and overall wellness. Developed by those with lived experience, WRAP provides a structured approach to identifying personal wellness tools and creating a plan to maintain well-being. Key Components: • Wellness Toolbox: A personalized collection of strategies and resources that promote wellness. • Daily Maintenance Plan: Guidelines for daily activities and routines that support mental health. • Identifying Triggers: Recognizing external events or circumstances that may impact well-being and planning appropriate responses. • Early Warning Signs: Noticing subtle internal signs of distress and implementing strategies to prevent escalation. • Crisis Planning: Developing a plan for others to follow during times when the individual cannot make decisions, ensuring safety and support. • Post-Crisis Planning: Strategies for recovery and returning to wellness after a crisis has occurred. Core Values: • Hope: Belief in the possibility of wellness and recovery. • Personal Responsibility: Taking ownership of one's wellness journey. • Education: Learning about oneself and available resources to make informed decisions. • Self-Advocacy: Effectively communicating needs and desires to others. • Support: Building a network of people who provide encouragement and assistance. Trained individuals facilitate WRAP and can be implemented in various settings, including mental health centers, peer support groups, and community organizations. It empowers participants to create a personalized plan that promotes resilience, reduces symptoms, and enhances quality of life.
